Cardiovascular Health Level in Older Age may associate with Cognitive Decline

In French population-based cohort study involving 6,626 individuals, an increased number of optimal cardiovascular health metrics were significantly associated with lower risk of incident dementia.  The metrics are defined by AHA, the American Heart Association, as a 7-item tools such as; 1)nonsmoking, 2)body mass index <25, 3)regular physical activity, 4)eating fish twice a week or more and fruits and vegetables at least 3 times a day, 5)cholesterol <200 mg/dL[untreated], 6)fasting glucose <100 mg/dL[untreated],and 7)blood pressure <120/80 mm Hg[untreated] These findings may support the promotion of cardiovascular health to prevent development of risk factors associated with dementia. More time spending in sitting is related to the cause of cardiovascular disease

A cohort study is published in the U.S. mentioning that prolonged leisure-time sitting (6+ vs <3 hours/day) is associated with higher risk of not only cardiovascular disease (including coronary heart disease and stroke-specific mortality),  but also cancer, diabetes, kidney disease and other diseases. This cohort study includes 127,554 men and women who were free of major chronic disease at study entry and among whom 48,784 died during 21 years follow-up, and supports that encouraging individuals to reduce sedentary time may provide health benefits. A MCG clinical study is accepted in the largest conference for Health Checkup centers in Japan.

A clinical study using MCG is accepted and will be presented in the 59th Annual scientific meeting of Japan Society of Ningen Dock, which is the largest conference in Japan for health checkup centers. In the study, the data of ECG and MCG for more than 300 patients are compared to CT and other cardiac examinations. The 59th Annual scientific meeting of Japan Society of Ningen Dock will be held in August 30th and 31st in Niigata, Japan, where Premier Heart exhibits MCG as an exhibitor. FFR-guided PCI strategy showed significantly lower rate of composite end point of death, myocardial infarction, or urgent revascularization.(FAME2)

This trial was conducted at 28 locations and 1,220 patients who are with stenosis >50%, comparing PCI + medical therapy to medical therapy alone. The 888 patients who are FFR<0,80 were randomly categorized in either group. At 5 years, the patients FFR-guided PCI group showed significantly lower rate than medical therapy only group, for composite end point of death, myocardial infarction, or urgent revascularization. MCG is the test to detect abnormality of heart cell without any invasion. It is useful for long term monitoring of the patients who had surgery or events without invasion.

The leading cause of death for diabetics

   About 422 Million adults were suffered from diabetes in 2014 worldwide, as reported by WHO in 2016. One of the leading cause of death in these patients is heart disease. According to the report, about 2.2 million deaths are from cardiovascular diseases, chronic kidney disease, and others related to higher-than-optimal blood glucose. Also, adults with diabetes have a two or three times higher rate of cardiovascular disease (CVD) than adults without diabetes. The study conducted in Japan revealed that the heart disease is the No.1 Killer of diabetes. This study follows 1939 diabetic patients in Osaka about 10 years, and the mortality rates per 1000 person-years were 31.35 for males and 21.99 for females. Among them, heart disease killed 19.5%, cerebrovascular disease 16.7% and renal disease 13.1%. Thus this study showed heart disease is the No.1 killer of diabetes. It is challenge for conventional imaging technology to find abnormality of small vessels

The ischemic heart disease caused by stenosis is called "structural ischemia". It can be found by CAG(Coronary Angiogram) in large vessel.  However, there are other cause of ischemia called "functional ischemia", which is sometimes hard to detect by imaging examination especially for small vessels. Conventional imaging examinations are not designed to detect such disease. Functional ischemia is the condition where heart muscle is less active mainly due to the supply of oxygen is insufficient by stenosis of large vessel. It is told that the blood flow of the coronary artery is said to be about 20% of the whole heart. Therefore, even if there is no stenosis in the coronary artery itself, there are cases where the cardiac function declines when the peripheral blood, inside of heart wall, inside of inner blood vessel and etc., are in poor condition. MCG study is presented at ESC Acute Cardiovascular Care 2018

MCG clinical study was presented at ESC Acute Cardiovascular Care 2018 in March 3rd, in Milan, Italy. This study was conducted in Myanmar, where the locations of the cath-lab are very limited.  MCG was compared with CAG, and it showed high sensitivity and specificity for the presence of severe CAD by CAG (≥ 75%). By Author, it is evaluated as “MCG could be used for prompt detection of severe CAD in order to start appropriate (PCI/Optimal Medical Therapy) treatments in the emerging country such as Myanmar where less medical facilities and skills are available.”

MCG Clinical Case is reported in Malaysia

The MCG case report is published in the issue of February of “MIMIS DOCTOR”, the famous trusted information source of health care in Malaysia. The author, Dr.Ebrahim reported  a case that MCG score is low even the stenosis is  recognized by CTA. However in this patient, he also recognized collateral circulation supplying enough blood to the heart cell instead of the main vessel, which may affect the MCG score is low. He concluded “Coronary artery computed tomography angiogram is currently used to assess coronary artery disease. Multifunction cardiogram is a new diagnostic tool in cardiac care to detect microvascular and cellular function of the heart muscles.” For details, please see the attached journal.
